grant application procedures

FOR REPLICATION AND PROFESSIONAL DEVELOPMENT GRANTS:

There are no application forms. To request a grant, an organization must submit the following:

Cover letter
Executive summary of no more than two pages
Proposal describing applicant agency, history, purpose for which funds are being requested, and evaluation plan
Data or other evidence demonstrating the effectiveness of the program or practice for the target population
A list and description of objectives by which the program will be measured and a description of expected outcomes
Copy of IRS determination letter
Budget for agency
Budget for project, including funding strategy
Copy of last audit
List of Board of Directors

 

Applications for Replication and Professional Development Grants are due by February 15 and August 15. Telephone or e-mail inquiries are welcome.  Grant decisions are generally made in November and May.

FOR EQUIP GRANTS:

Guidelines and application forms specific to the Early Childhood Quality Improvement Program (EQUIP) may be requested by calling 312/641-5765 or 312/641-5766. Applications are due by June 1. Grant decisions are made in July.  Grants are paid by EQUIP grantmaking partner, Bright Promises Foundation.
